Is a king of Egypt a Pharaoh by definition? Is a king of Rome an Emperor? Is a king of Judea a Herod?

King of Egypt = Pharaoh
King of Rome = Roman Emperor
The King of Judea is not necessarily a Herod though. But the other two terms are interchangeable. Herod is a family name, not a title.

Perhaps not necessarily, but historically all Judean kings were Herods, including those who married into their position.

I would lean towards them still not being Herods as what I'm finding is that they had a dynasty, but not everyone who ruled Judea is a Herod.

Being a king, and being Egyptian doesn't mean your an Egyptian King (even though in most cases it would be accurate). The same could be said for the other examples you asked about. We already have an example of this in the game with Abimelech who is a King and an Israelite but not an Israelite King.
For example, if you were to choose Israelite as the nationality for Kings of the Earth, that does not make him a King of Israel so that King Amaziah can use his CTB ability on him.
