If I (as the Tournament director) take a Promo, do I have to put my name on the Address list?

Yes, you need to sign the address list as a participant, as only participants get the Promo.  Otherwise when you send it back with the remaining promos, Cactus sees a difference in the number of names (participants) and promos, and they'll have to wonder where the 'extra' promo went ;)

So yeah, sign in as a participant, even as the tournament host.
